Welcome to our staff, and thank you for choosing UT Medical Center as your place of employment. As a new employee, you will need to provide us with several forms in order to complete your benefit and payroll enrollment. The following links are to forms that you will need to print, complete and bring with you on your first day of employment. Also you will find below, a link to our Benefit Summary, which will give you an overview of all the fringe benefits and how you may participate.
All new employees are required to attend Orientation, which is 1½ days and will be followed by your departmental orientation process. Benefit and payroll enrollment is on day 1 of Orientation, and you should have these forms with you at that time.
- Benefit Summary Sheet:
- Personnel Action Form: This form is utilized to capture the employee data information. Please complete the top portion only. We do not need you to enter Accounting Unit nor Employee Number.
- Employment Eligibility Verification (I-9) Form: Normally this form is completed when you visit Human Resources for your medical screening. If this was not completed during that visit, please complete Section 1 only. Then, your first day of employment, we must see two forms of identification on the List of Acceptable Documents. You will be ineligible to work if you do not provide these documents within 3 business days of your employment date.
- Direct Deposit Form: We require direct deposit for all employees. An employee has the option of a primary and a secondary account. You may choose a checking account or savings account or both. The maximum number of accounts we will direct deposit is two per employee. Valid documents for direct deposit are a voided check or a letter from your bank with account number(s) and bank routing number.
- W-4 Form: This is the Employee’s Withholding Allowance Certificate. Please complete the lower portion of this form to return to Human Resources. The top portion of the form is a Personal Allowance Worksheet so it is optional worksheet to calculate your deductions. If you claim Exempt from taxes, we do ask that you speak with our Payroll Department your first day of employment. We want to ensure we are accurately deducting taxes from your payroll checks.
- Benefit Eligibility Criteria: An employee must work at least 20 hours per week in order to be eligible for benefits. You must be assigned a regular position and work one full calendar month before your medical, dental, vision, life insurances, long term disability, short term disability benefits begin. (Please make sure you have all dependent social security numbers and birthdates when completing your benefit forms.) After you work 1000 hours, UTMC will contribute to our 403(b) retirement plan. We have a 6 year graduated vesting schedule so will need to work 6 years to be 100% vested in our 403(b) retirement plan. Any contributions you choose to make, deferred income, are vested 100% immediately.
- Leave Benefits: An employee accrues annual and sick leave each pay period. We do not have a waiting period to utilize your accrued time. Rather, your supervisor must approve your time off in advance. We do offer personal leave days and they are prorated your first year of employment.
