When you visit the transplant center for a follow-up or annual visit, it is important that you bring your prescription information and a list of all the medications you are taking. Be sure to include the dosage and pharmacy you are using. Making sure your medications are accurate is very important to the life of your kidneys.
If you are making your annual visit, you must also bring with you a 24-hour urine sample. You can obtain a urine jug from the lab or you may collect urine in a gallon-size water just (make sure the cap screws on). Keep your urine in a refrigerator until your visit.
