- Volunteer Application Form - This can be completed online
- Medical Release Form- must be mailed or faxed
- Health Screening - must be signed
- Drug Disclosure - must be signed
- Background search authorization- must be signed
- Additional Information Sheet
Volunteer Application Form (complete online)
You must print and sign: Medical Release Form, Additional Information sheet, Health Screening, Drug Disclosure and Background Search Authorization. Bring these forms with you at the time of your interview except for the Medical Release form which must be mailed or faxed. For additional information call 305.9515.
For Junior Volunteers!
Junior Volunteer Application Form
Junior Volunteers must also print and mail in a Parent Permission Form as well as a Medical Release Form. The annual deadline for junior volunteer applications is April 15.
