Prepare for Your Consultation

Prepare for Your Consultation

A consultation appointment is an office visit with one of our board certified sleep physicians. During the evaluation, your doctor will review your medical history and perform a physical exam.

You will be asked to discuss your sleep complaints as well as any other related conditions. Your physician will work with you to develop a treatment plan and may recommend a sleep study.

It is always wise to prepare a list of questions and key issues you would like to discuss before your appointment to ensure all of your questions are answered.

Before Your Appointment

In order to ensure your visit goes smoothly, please:

  • Verify your insurance coverage and bring your current insurance card and a photo ID.
  • Complete a sleep history questionnaire before arriving to the Sleep Center.

If you are a new patient, please complete this New Patient Questionnaire. If you have previously been seen by a sleep specialist, please complete this New Patient Questionnaire – Previously Seen. If you do not want to complete the questionnaire online, please print, complete and bring the attached hard copy to your appointment.

When to Arrive

Please arrive 15 minutes prior to your scheduled appointment for registration purposes.

If you have difficulty staying alert while driving please make arrangements for transportation to and from your appointment.

What to Bring

  • Any previous sleep study results (if available)
  • Your CPAP machine, mask and any other equipment (if applicable)
  • A list of your medications

Follow-Up Visit

After your initial consultation you will be scheduled for a follow-up visit. This visit can be to discuss the results of any testing and/or to monitor your response to treatment.

You will be asked to fill out a short questionnaire when you arrive at the clinic, or if you prefer, please complete the Sleep Follow-Up Questionnaire about a day before your follow-up appointment. Your sleep physician will review your responses at your appointment.
