The referral staff at each location will work with your patient to schedule an appointment.
Before your patient’s first appointment, we will need pertinent medical records, from the original diagnosis to the present including:
- Clinical notes
- Pathology, radiology, lab, and surgical reports
- Treatment summaries
We recommend your patients follow up with the practice as well. The physician will keep you and your patient apprised of treatment plans.
